Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2, allowing for multiple and diverse meta-epidemiological studies to examine the effects of treatment across trials that employ different levels of pragmatism, as well as other design features.
Background
Pragmatic studies provide real-world evidence that can be used to make clinical decisions. However, the use of the term "pragmatic" is inconsistent and its definition as well as assessment requires clarification. Pragmatic trials are designed to inform clinical practices and policy decisions, not to confirm a physiological hypothesis or clinical hypothesis. Many RCTs that do not meet the criteria for pragmatism but contain features in opposition to pragmatism, have been published in journals of varying kinds and incorrectly labeled pragmatic. The development of a PRECIS-2 tool that provides an objective and standardized assessment of pragmatic features is a first step.
Methods
In a pragmatic study, the aim is to inform clinical or policy decisions by showing how an intervention could be implemented into routine care. This is different from explanatory trials that test hypotheses regarding the cause-effect connection in idealized situations. Results
While the definition of pragmatism does not require that all clinical trials are 100% pragmatic there are benefits to including pragmatic components in trials. These include:

The overall score for pragmatic systematic reviews was lower when the domains of organization, flexible delivery, and following-up were combined.
It is important to note that a pragmatic trial doesn't necessarily mean a low-quality trial, and there is an increasing number of clinical trials (as defined by MEDLINE search, however this is neither sensitive nor specific) that employ the term "pragmatic" in their abstract or title. The use of these terms in abstracts and titles may suggest a greater awareness of the importance of pragmatism, but it isn't clear if this is evident in the content of the articles.
Conclusions
As appreciation for the value of real-world evidence becomes increasingly popular and pragmatic trials have gained traction in research. They are randomized trials that compare real world care alternatives to clinical trials in development. They include patient populations that are more similar to those who receive treatment in regular care. This approach can overcome the limitations of observational research such as the biases associated with the reliance on volunteers, as well as the insufficient availability and codes that vary in national registers.
Other advantages of pragmatic trials are the ability to utilize existing data sources, and a greater probability of detecting significant changes than traditional trials.
